All payer rate setting seems a great idea. But how do we set the rates, given the detachment of costs from quality of care and their already inflated prices?
The nurse practitioner idea is a good one, but it would be far better if they operated in a clinic with a doctor always nearby to call on for second opinions and difficult cases. We should be wary of nurse only medical facilities for good reason. The doctors that assist them, call them super GPs, would have more interesting cases and a better ability to earn. And fewer would be needed. Win win win?
